Efficacy for Ventricular Tachyarrhythmias Using Ambulatory Monitoring and Electrophysiological Techniques
نویسنده
چکیده
Sustained ventricular tachycardia and ventricular fibrillation are life-threatening arrhythmias with high rates of recurrence.1'2 Empiric antiarrhythmic drug therapy has a low success rate for preventing recurrences of these arrhythmias.3 Therefore, the choice of drug therapy has centered around two methodologies to predict drug efficacy: programmed ventricular stimulation with serial drug testing and suppression of ambient complex ventricular ectopy and nonsustained ventricular tachycardia detected by ambulatory monitoring recording and exercise stress testing. There is evidence that both approaches can successfully identify appropriate prophylactic antiarrhythmic drug therapy for sustained ventricular tachyarrhythmias.3-6 The study reported in this issue of Circulation7 was designed to compare the effectiveness of these two methodologies in determining antiarrhythmic drug treatment for such arrhythmias and to identify any clinical characteristic that can help predict drug efficacy in the context of these different methodologies.

متن کاملFlecainide and amiodarone: combined therapy for refractory tachyarrhythmias in infancy.
OBJECTIVES This study assessed the safety and efficacy of combined flecainide and amiodarone therapy in controlling refractory tachyarrhythmias in infants. BACKGROUND Single-drug as well as standard combination medical therapy for tachyarrhythmias in infants sometimes fails. In those cases, one may consider interventional therapy. However, this option may carry a high risk of morbidity and mo...
متن کاملComparison of 24 versus 12 hours of ambulatory ECG monitoring.
In order to assess the additional information obtained from 24 hours compared to 12 hours of ambulatory ECG (electrocardiogram) recording, we analyzed 72 ambulatory ECG monitoring tapes in which arrhythmias were present. In all cases the second 12 hours included the entire period of sleep. Only 38 of 233 (16 percent) episodes of arrhythmias, frequent premature ventricular contractions (PVC's), ...
متن کاملFailure of ambulatory electrocardiographic monitoring to predict results of programmed electrical stimulation. Studies in patients with clinical ventricular tachyarrhythmias.
To test for an association between the results of ambulatory electrocardiographic monitoring (AEM) and programmed electrical stimulation (PES), or whether other factors better predict the results of PES, 57 patients (36 male and 21 female patients) presenting with either ventricular fibrillation (49 percent; 28/57) or sustained ventricular tachycardia (51 percent; 29/57) were studied. Each pati...
متن کاملDiagnosis of broad complex tachycardias with ambulatory monitoring of atrial electrography.
Simultaneous 24 hour ambulatory monitoring of the surface electrocardiogram and the intra-atrial electrocardiogram was evaluated in eight patients with broad complex tachycardia. The technique using a J shaped atrial pacing wire permitted a distinction between ventricular tachycardia and supraventricular tachycardia in all eight patients. Formal electrophysiological testing would still be requi...
